Registration prior to the ECAWA 2010 State Conference is essential, and delegates whose registrations are received by the Friday the 17th of September will be entered in an extra prize draw, so don’t wait – register now
The ECAWA 2010 Conference Registration Form may be downloaded as a PDF and you may then register in one of several ways.
1. You only need to sign the registration form if you are registering as a non member or if you are paying by credit card.
2. If you download, save and then open the PDF form with Adobe Acrobat Reader (a free download) you will be able to fill in the form by entering text, checking boxes, etc. If you then click on the ‘Submit’ button at the top of the form (only visible with Acrobat Reader) you will be prompted to either send the form automatically via your email client, or to save the form to send it manually as an attachment via webmail to email@example.com The latter option will also allow you to print a copy of your completed form.
3. When you ‘Submit’ the form it is likely that you will receive a warning that the form has also been distributed to others.
Do you need an invoice sent to your school?
If you would like an invoice sent to your school please indicate this when you complete the form. Include your school order number (if applicable) in the text area labeled ‘Other’ in the payment section of the form. ‘Submit’ or email the form to firstname.lastname@example.org OR post the completed form to PO Box 297 Claremont, 6910.
Paying by direct deposit?
If you wish to pay by direct deposit from your account to the ECAWA bank account and you are already an ECAWA member, make the payment using the details specified on the registration form, complete the form and use the ‘Submit’ button to send the form to email@example.com If you are registering as a non member, print and sign the form and either scan and email the form OR post it to PO Box 297 Claremont, 6910. Please note that Direct Deposit is ECAWA’s preferred form of payment.
Paying by cheque?
If you wish to pay by cheque, complete the form, print and sign the form (if necessary), and then post the form with your cheque to PO Box 297 Claremont, 6910.
Paying by Visa card or Master Card?
If you wish to pay by Visa card or Master Card complete the form, print and sign the form, and then either post the form to PO Box 297 Claremont, 6910 OR scan the form and email it as an attachment to firstname.lastname@example.org
ECAWA and GST
ECAWA is now registered for GST, and GST is payable on all ECAWA membership fees and Conference registration fees. The ECAWA 2010 Conference registration form will become your TAX INVOICE when it is completed.
